Here is what you need to complete in time for the meeting:

1. Please confirm your travel plans

2. If you have not already done so, please complete an on-line application (team leaders are exempted) - please do so as soon as possible. http://healthyunnan.org/Application.html

3. Please make sure that your passport is current and that you have additional visa pages

4. Plan on attending the team meeting on January 17-18 2009 at our home: 909 Melbury Court, Redlands, CA 92373

a. The meeting on Saturday January 17th will start at 1:00 PM and will end at 9:00 PM. We will provide dinner.

b. The meeting on Sunday January 18th will start at 12:00 PM and will end at 5:00 PM. We will provide snacks.

c. You must bring to the meeting:

i. A check written to MPPC (Menlo Park Presbyterian Church) for $2080 if you are going to Kunming and Zhongdian or 1980 if you are just going to Kunming

ii. A check to Best Holidays for $60 for your visa

iv. Your passport

v. A completed Chinese Visa form (see Health Yunnan website for form). Please check the tourist box.

d. Attendance at the team meeting is essential so that you understand the team leadership structure, values, plans, cross-cultural challenges and can complete the team planning. We will be asking you to accept a team task.
