Team Application
These are the items that need to be completed for your application:
- TEAM APPLICATION (CLICK ON "TEAM APPLICATION" BELOW). BE SURE TO CLICK "SEND" WHICH IS LOCATED ON THE BOTTOM RIGHT.
- SUBMIT YOUR CV OR RESUME TO PETER YORGIN (pyorgin@yahoo.com).
- ARRANGE FOR AN INTERVIEW IN PERSON OR BY PHONE WITH BOB ARRINGTON (rna32000@yahoo.com) or (650) 343-4994 AND PETER YORGIN (pyorgin@yahoo.com) or (909) 266-1705
- ONCE YOU HAVE INTERVIEWED WITH BOB AND PETER WE WILL LET YOU KNOW IN ONE WEEK IF YOU ARE A GOOD MATCH FOR THE TEAM. YOU ARE A TEAM MEMBER IF YOU HAVE RECEIVED NOTIFICATION FROM THE TEAM LEADERS.
ALL OF THE INFORMATION ABOVE NEEDS TO BE SUBMITTED NO LATER THAN March 31, 2009
Team leaders do not need to submit an application, but need to submit the items below.
Once accepted on the team, you will need to send the following to either Peter Yorgin, MD (909 Melbury Court, Redlands, CA 92373 - pyorgin@yahoo.com) or YOU CAN BRING THIS TO THE TEAM MEETING.
- A CHECK TO MENLO PARK PRESBYTERIAN CHURCH (MPPC) FOR THE AMOUNT OF THE TRIP (This will be considered a donation to MPPC and will be tax-deductable)
- YOUR PASSPORT
- TWO PASSPORT PHOTOS
- A COMPLETED CHINESE VISA FORM CHINESE VISA FORM (PDF)
- A SCAN OR COPY OF YOUR PROFESSIONAL LICENSE
- A SIGNED MENLO PARK PRESBYTERIAN CHURCH RELEASE FORM
ALL OF THIS INFORMATION NEEDS TO BE SUBMITTED NO LATER THAN MARCH 31, 2009.
Refund information: If the trip is cancelled prior to April 8th, all except $350 will be refunded. If your trip is cancelled between the dates of April 8th and May 8th, all except $600 will be refunded. We will work with you to obtain a refund from the airlines and our insurance provider, however, the additional money may not be refundable.
